Connected & Prepared Disaster Plan (CaP)

Gold Coast Primary Health Network (GCPHN)

A person-centred emergency preparedness program that helps individuals plan for medications, medical equipment, healthcare, support services, communication, transport and other essential needs during emergencies and disasters.

About this service

Connected & Prepared is an interactive, person-centred program that supports individuals and communities to prepare for emergencies and disasters before they happen. Using a personalised action plan, participants explore how emergencies may affect their health, wellbeing and daily life, including access to medications, medical equipment, healthcare, support services, transport, communication, utilities and social connections. The program encourages participants to identify their individual needs, strengths and support networks, helping them develop practical strategies to maintain their health, safety and independence during disruptions. Connected & Prepared is suitable for individuals, carers, people living with disability, older adults, people with chronic health conditions, support workers, health professionals, community organisations and anyone wanting to be better prepared for emergencies. Workshops are practical, interactive and tailored to the needs of participants, building confidence, resilience and stronger community connections.
